Compression leg sleeves work by applying external pressure to the lower leg, which helps veins push blood back toward the heart and reduces blood pooling in the legs. They’re used for two very different purposes: managing venous and lymphatic swelling as a medical tool, and supporting recovery after exercise in sports settings. The distinction matters because the evidence is much stronger for one than the other.
The Mechanics: Graduated Pressure Explained
All compression sleeves rely on the same principle. External pressure helps your veins push blood back toward the heart, and graduated pressure—strongest at the ankle or lower calf, then easing as it moves up the leg—is the design that supports venous return most effectively. That pressure gradient is what lowers edema risk by preventing blood from pooling in the lower leg.
Here’s what that looks like in practice:
- Stronger at the bottom: Pressure is highest at the ankle and lower calf, which gives blood a “push” upward.
- Lighter toward the knee: Pressure decreases as it moves up the leg, encouraging upward flow.
- Static fabric, not air: A sleeve is a fixed elastic garment; an inflatable intermittent pneumatic compression (IPC) device uses air and works entirely differently.
What the Evidence Actually Supports
Clinical evidence is strongest for compression stockings or sleeves as a treatment for venous symptoms and swelling. Around 20 mmHg of pressure on the distal leg increased venous blood-flow velocity and prevented leg swelling after prolonged sitting or standing in a review of clinical evidence. That makes sleeves genuinely useful for long flights, extended desk work, and chronic venous disease—where they can alleviate symptoms and improve quality of life.
